Predictors of successful discharge in a residential treatment program for the mentally ill homeless were evaluated. The charts of all program residents during a single year were reviewed (N = 161). Data were collected on demographic, clinical and intake variables. Severity of Psychiatric Illness (SPI) and Acuity of Psychiatric Illness (API) questionnaires were retrospectively completed for each admission. Outcome variables included Planned Discharge, Monetary Linkage, Housing Linkage, Psychological Management, Psychological Stability and Global Functioning. A logistic regression (alpha level =.05) was performed. Across outcome variables, strong positive predictors included: the client's length of stay in the program; and use of external case management. Strong negative predictors included: dual diagnosis status; history of severe residential instability; and higher scores on the API-Nursing section (indicating greater need for nursing contact). Clients receiving individual psychotherapy were one-fifth as likely to have a planned discharge as were other clients. Clients receiving group psychotherapy were seventeen times as likely to demonstrate improved global functioning as were other clients. Findings were largely consistent with the existing empirical and theoretical literature on care for the mentally ill homeless. The strong impact of the |length of stay| variable suggests that residential treatment can be of great use to homeless clients. Indicated is the need for residential treatment programs that will keep clients engaged for longer periods of time.
